Initatial Experience of Iris-Transobturator Tape Procedure for Female Stress Urinary Incontinence

Abstract
Purpose: Intrapelvic route of retropubic tension-free vaginal tape (TVT) procedures exposes the patient to a number of complications. Transobturator tape (TOT) procedure is the newly developed method to decrease these complications. We assesed the initial short-term efficacy and safety of the Iris-TOT mid-urethral sling.

Materials and method: From September 2005 to September 2006, 40 patients with stress urinary incontinence were operated using Iris-TOT. Then, they were evaluated in terms of postoperative success rate, change of voiding symptoms, improvement of urgency and urge incontinence retrospectively.

Results: Mean follow-up was 257 days. Mean operating time was 36 minutes. Average incontinence period was 6.2 years. Success rate was 95% and percentage of satisfied and tolerable patients was 87.5%. Postoperative maximal flow rate and voided volume, flow time were decreased and postvoid residual urine volume was unchanged. (p=£¼0.05, 0.01, 0.46, 0.04, respectively) Postoperative daytime frequency, nocturia were decreased but interval of voiding was increased. (p=£¼0.05, £¼0.05, £¼0.05, respectively) Complications were 2 vaginal wall perforations and 1 vaginal erosion, 1 vaginitis, 1 de novo urgency.

Conclusion: Iris-TOT procedure has fewer complications and similar success rate than retropubic TVT procedure.
This procedure will need long-term follow-up and become standard method for stress urinary incontinence to reduce
complications of retropubic TVT procedure.
